The woman had extra supervision when she was confused. After the incident, the accommodation made a lex-Maria report to the Swedish Health and Care Inspectorate (Ivo).
It was when the woman was traveling in the accommodation in the wheelchair that the staff opened a front door. The woman must then have rolled out and fallen down five steps. According to the report, the staff “almost got hold of the wheelchair handle”.
The woman suffered fractures and bleeding in the skull. She was taken to hospital but was quickly returned to the accommodation again. She died four days after the accident.
