A woman in her 60s died after receiving too high a dose of chemotherapy in connection with treatment for her cancer.
The health care failed to check a sample, which led to the doses in the treatment not being reduced according to routine, writes Kalmarposten.
The case has now been reported according to lex Maria by the chief physician at the County Hospital in Kalmar and the region states that measures have been taken to reduce the risk of it happening again.
