A player randomly received an item with the current highest item level in Diablo 4 and got really lucky with an upgrade with the properties. Here you can find out when an upgrade is also worthwhile for you.
If you find items that have good properties but could still use a push, your trusted jeweler and blacksmith will help you further. Every upgrade you do to your items there increases item power by 5 points.
A piece of gear with the highest item level was reported on Reddit on June 23, which has an item power of 870 after the upgrade.
“Then this trophy came up”
What is this item? The item is the Infernal Edge two-handed sword, which has an item power of 845 and is featured by u/5altwaterCowboy in his post. There he writes that he had nothing better to do at level 100 and farmed “Almunn”.
“Almunn” is a unique monster in Diablo 4 that grants a special item when killed. There are several of these in the open world, as we have listed here.

The characteristics of the two-handed sword are also quite good. The sword not only has a high damage per second with 3,022, but also a double boost for additional critical hit damage (+52.5%, +63%), a plus of 93% for the damage to injured opponents and +72 to all stats

How does he get the high item power? The item has an initial item power of 845, which is pretty high. 5altwaterCowboy went to the blacksmith with his new two-handed sword and upgraded the sword 5x.
Each upgrade increases item power by 5 points, as well as base stats such as armor or damage per second. This gave him an item power of 870.
According to Icy Veins, it is speculated that the drops from special elite enemies are worth up to 850. However, there have been no reports about this so far (via icy-veins.com).
When is an upgrade worthwhile?
Is an upgrade always worth it? Upgrading items is worthwhile if you have an abundance of materials and want to take the boosts with you, and if you are sure you want to keep the items. We recommend this if you’re close to endgame and starting at around 750 item power.
If you find legendaries that don’t have good traits but good legendary aspects, you can extract them from the occultist. Note that you can only use the extracted aspects once.
Non-weapon gear doesn’t change quite as much after 725 item power. This is related to the breakpoints.
When upgrading, you should keep an eye on the breakpoints in addition to your materials and your gold, because: if you upgrade items, they should rather be located close to the level limits in order to experience a noticeable boost.
