In Last Samurai, however, he actually almost lost it in an accident. Hardly more than an inch separated him from the decapitation by co-star Hiroyuki Sanada.

On TV: Tom Cruise almost lost his head on the Last Samurai
The Japanese actor (Wolverine: Way of the Warrior) told The Guardian in 2004. Cruise, who begins the film as an American veteran campaigning against the samurai, rode a mechanical horse in one scene. But that didn’t throw him off as planned due to a defect. He also couldn’t dodge his co-star’s sword, that swung in his direction. Sanada explains:
Tom’s neck was right in front of me and I wanted to hold my sword open, but that’s hard with one hand. […] The crew screamed from all sides. You thought Tom’s head would fly through the air.
Apparently he could only “half an inch“Stop in front of Cruise’s neck. The experienced martial artist Sanada (via Outnow) managed to end the incident without bloodshed. He doesn’t name the exact scene of the almost decapitation. It’s very likely but it is the first battle sequence, in which both stars are on horseback.
