Perth County Ontario Provincial Police, Wellington County OPP, Perth East and Mapleton fire services, Perth County paramedics and Ornge air ambulance responded to the crash just after 4 pm on Line 86 between Road 121 and Road 124, north of Millbank.

The 64-year-old was pronounced dead at the scene, and the injured person was airlifted to a trauma center. Four others were taken to a local hospital.

Perth Line 86 between Road 121 and Road 124 was closed until Sunday morning as members of the West Region OPP Traffic Incident Management and Enforcement Team assisted Perth County OPP with the investigation.

While the cause of the crash is unknown, Perth County OPP are taking part in an annual Easter weekend traffic initiative that starts Friday and ends Monday night.

The Easter Weekend Campaign focuses on the need to buckle up to save lives. Perth OPP officers will be focusing on awareness through enforcement and public education by stopping vehicles that contain a driver and/or passengers not wearing their seatbelt.

Officers will also be looking for other unsafe driving behaviors including distracted driving, driving under the influence of alcohol or drugs, speeding and other forms of aggressive driving.
