During a nude scene in X-Men: Origins, Hugh Jackman, claws in hand, has to run in the dark. He comes across young women on his way and, by reflex, he wanted to hide his private parts, forgetting his claws, and injured the inside of his thighs.
These actors got injured in the middle of a scene, and it could have been serious.
