A woman in her 60s died of heart failure that the healthcare system missed.

The woman was sent on referral by a specialist clinic to her health center for investigation of, among other things, heart failure. But based on the woman’s story, the doctor judged that the patient’s symptoms were not due to heart failure – and thus no full investigation was started.

Four months later, the woman died of cardiac arrest. The autopsy showed that the woman had acute heart failure and atherosclerosis with narrowing of the coronary arteries in the heart.

The incident has been reported to the Inspectorate for Care and Care according to lex Maria, Region Sörmland writes in a press release.

The healthcare provider has carried out an internal investigation into the course of events. According to the region, risk prevention measures in terms of routines, information and training must be taken to prevent something similar from happening again.
