The attending physician is the doctor of reference for the patient. At the heart of the health pathway, the designation of this also allows you to be better reimbursed. But how can you change your treating doctor if necessary?

The Health Insurance website reports that a treating doctor must be declared if you are in one of the following three situations:
- You do not yet have a treating doctor;
- You wish to change and declare another doctor;
- Your current treating doctor ceases his activity, changes profession or moves.
In these cases, you have two options for declaring or changing your treating doctor:
Declare your treating doctor online
This process can be carried out online by the doctor you have chosen, with your agreement. In practice, you simply need to present your vital card during a consultation at your office. The doctor in question transmits your declaration directly to your primary health insurance fund. Simple and quick, there is no form to fill out or mail to send.
Declare your attending physician by mail
For those who wish, it is also possible to complete the process by mail by downloading form S3704 from the Health Insurance website. You will need to complete it and have it signed by the attending physician of your choice. Then give it to your health insurance fund.
Good to know : the change of treating physician can be done as many times as necessary without the need for justification.
