What makes the boss so tough? In the challenge, you fight against the Echo of Lilith, who, after her defeat in Act 6, returns as “Hate Incarnate” – with significantly more lives and new abilities.
You’ll have to fight her and some minions over several stages, using mechanics like a boss from an MMORPG. She keeps disappearing and covers the whole platform with skills that you have to avoid.
Combat requires finding an optimal balance of offense and defense in your build while still being mobile enough to dodge. One of the best players in the world, wudijo, already tried the boss at level 80, but lost there several times after more than an hour (via YouTube).
Although there are already the first players at level 100, even in hardcore mode, nobody can compete against the Echo Lilith right now. Because the challenge is locked.

Blizzard bans “Pinnacle Boss” due to loot issues
In the official forum, Community Manager Adam Fletcher writes that the team is currently working on fixing a few items related to this boss fight. That was on June 6th, the official release of Diablo 4.
Fletcher does not write how long this will take or which items are specifically involved. However, a helmet should be available from the Lilith Echo, which will be the best item for almost all classes. Whether there are other loot sources for this has not yet been confirmed.
The team will provide an update once more is known. However, most players should not even be close to the challenge anyway. Leveling to level 100 will take several days for the average Joe, although some elite players complain that there isn’t enough to do:
