To better support people with cancer and their loved ones, the League is launching a new personalized support system for people with cancer. Entitled Au fil de la Ligue, this program is based on the sharing of patient experiences and peer support, with the cooperation of healthcare stakeholders, from hospital to home.
This new system will include the establishment of several types of reception areas with human and competent care, facilitated by the use of a digital tool. The Au fil de la Ligue system will be deployed across the region from 2025.
Offer a new personalized journey for patients and their loved ones
“Having the right information to manage your illness is a real blessing. Until today, the patient constructed his own experience, without it being able to be shared and useful to others. We designed the system over the course of the League to combat wandering in the care pathway and the loss of experiential knowledge. This embraces all areas of need of a sick person and their loved ones and a geographical and practical “go” towards people affected by cancer who do not necessarily have the desire, the strength and the means to go fishing for information to get through the illness as best as possible” indicates Dr Philippe Bergerot, volunteer president of the League Against Cancer.
The Au fil de la Ligue system offers continuous and personalized support for the sick person and their loved ones from the hospital to the home, with tailor-made solutions, meeting the specific needs of each territory, each committee and each partner. It is co-constructed by all the stakeholders in the care pathway: patients, caregivers, paramedics, institutions, companies, whose experiences and expertise it brings together.
Places for exchange and information
The system relies on places to support and inform:
- The Au fil de la Ligue spaces are places of calm and support located in healthcare establishments and in the city, with all the information necessary to experience the best possible care journey, in a serene and caring setting.
- The Au fil de la Ligue trolleys – ultra light and easy to handle – will circulate in healthcare establishments and will, for example, be available in the waiting room to provide information and improve patient comfort.
- The Au fil de la Ligue vans will connect isolated areas with prevention and supportive care services, with doctors on board.
The Au fil de la Ligue spaces and vans will be staffed by a health coach trained at the League against Cancer training school, to guarantee care that is both humane and competent, enriched by feedback from experiences of patients themselves.
Kommunity to streamline the patient journey and assess the quality of care offered
The Kommunity application will be the cornerstone of this system for sick people and the community of caregivers who are involved in their life journey. Thanks to it, sick people will have access to practical information; it will also make it possible to collect the experiences and experiences of sick people… These testimonies evaluating the quality of care will be the subject of an anonymized analysis shared with the health establishments concerned to enable them to continually improve the quality of the care. care and facilitate the link between community medicine and the hospital.
Thanks to the commitment of the pilot committees, the League against Cancer has a goal of deploying around forty spaces on French territory, from 2025. Five committees: Calvados, Isère, Loire, Loire-Atlantique, Maine-et-Loire volunteered to experiment with mobile spaces and work on the operating charter of the future system.
