64-year-old HG, living in Balıkesir, applied to a hospital with complaints of pain and swelling in his abdomen. As a result of the examinations, it was stated that the old woman had stomach bleeding. He experienced the first shock when he was referred to Izmir Katip Çelebi University Atatürk Training and Research Hospital for further treatment. As a result of the tomography performed here, it was diagnosed that HG had a tumor that weighed approximately 10 kilograms, was 38 centimeters long and 26 centimeters wide, extending from the lower part of his abdomen to under his heart. There, he was examined for stomach bleeding and was referred to us for further treatment. When the patient came to us, he was weak, tired and anemic. We immediately requested a CT scan. He had a palpable mass in his abdomen. He had a very large mass, approximately 38×29 centimeters, in what we call the pelvis, that is, extending from the lowest part of the abdomen to below the heart. We made preparations, thinking that this would be a malignant tumor. “We made the patient ready for surgery within a few days and performed the surgery,” he said.

Emphasizing that it is a rare case in the medical literature, Prof. Dr. Dilek said:
“It was a difficult surgery. We went into surgery with our blood supplies. We thought before that it was caused by the stomach, but it was hard to believe. Because it was too large a mass for such a tumor to develop in the stomach.”

Stating that because the mass was very large, a large opening was made in the abdomen and the mass was removed together with the spleen, Dilek said, “The mass was intertwined with the spleen. It was stuck to the diaphragm and we removed it together with the diaphragm. We removed the entire stomach. We completed our surgery by creating a new path between the intestine and the esophagus. “It was a surgery that lasted about 4 hours,” he said.
Expressing that the patient’s condition is currently good and they hope that it will get better, Dilek said that they expect the patient to be discharged within 7 days.

Providing information about the examination process after the mass is taken to the pathology laboratory, Prof. Dr. İzzetiye Ebru Çakır said, “A mass with dimensions of 38,29,13 centimeters and a weight of 10 kilos came to our laboratory. Thanks to the pathology result, our patient will be able to receive treatment. However, these processes continue in pathology for approximately 3 weeks. All surgically removed materials must be subjected to pathological examination. “This type of intra-abdominal mass may be a gastrointestinal stromal tumor (GIST) originating from the stomach or other mesenchymal tumors such as dedifferentiated liposarcoma,” he said.
