64-year-old İlhan Tanrıver, who has been suffering from prostate disease for many years and could not go to the doctor out of fear, applied to the hospital when his prostate reached an unbearable size. Dr. Önder Çinar said, “Mr. İlhan came to our polyclinic with a catheter attached with the complaint of ‘inability to urinate’. He had prostate disease for a long time but had not received treatment. As a result of the tests, we saw that the patient’s PSA (Prostate specific antigen) was high. For this reason, we did all the examinations, including MRI (MRI). We detected ‘benign prostate’ in him.
We planned the surgery to ensure that the patient could urinate comfortably. Normally, 350 grams of prostate is the choice of ‘open surgery’ for most physicians. With today’s technological developments, especially laser technology, surgery can be performed in which the prostate can be completely removed by entering the urinary canal, which we call holep, and the patient does not experience any bleeding or pain. With this surgery, the patient does not have to stay in the hospital for a long time. “Usually, the patient who stays under observation for 1 night is discharged,” he said.

Assoc. Prof. emphasized that the closed method surgery they used for the case is also very important in terms of medicine. Dr. Çinar said, “When our patient came to us, he had a prostate of 350 grams. We discussed all treatment options with him. We also talked about open surgery, but he wanted to have endoscopic closed surgery. Since we performed closed surgery, the patient did not need to have a catheter, drain or hospitalization for a long time. It has been 2 days since the surgery and we will discharge our patient in good health. The prostate increases with age. We expect those over the age of 50 who have difficulty urinating to have regular examinations. Prostate can be detected early during annual check-ups. İlhan Bey’s problem was related to his inability to urinate. There was also bleeding in the urine. We performed a good surgery. There was no problem after the surgery.
I graduated from medical school in 2002. I have been a physician for 22 years. I have been an expert since 2012. I have been in the urology community for 16 years. We have performed prostate surgery of this size, 350 grams, using the open method. We may come across 1-2 patients a year on whom we perform open surgery. I don’t remember a surgery of this magnitude being performed using the closed method in our region. I don’t think a prostate larger than 350 grams has ever been removed using a closed method in prostate operations performed to date. Regarding this, I think our case is the first case. It is possible to repair large prostates with the closed method, regardless of the size of the prostate. Holep surgery has an equal success rate with other surgical methods. “That’s why I would like to remind our patients who have large prostates and are afraid of surgery that surgery can be performed using a closed method,” he said.
