Who is this NPC? Volo is an NPC in Baldur’s Gate 3 that you first encounter in the Emerald Grove. He is an author and bard who tends to be dramatic and spread lies.
While you can’t take him as a companion in battle, he can give you one of the best buffs in the game thanks to eye surgery, allowing you to see invisible units.

It’s actually worth considering getting an eye gouged out by Volo. Even if it sounds terrible, you get a huge advantage.
You may lose an eye, but you get a fancy replacement eye that lets you see invisible units. You gain no disadvantage, unlike Ethel Eye. For Volo’s Eye, you only have to endure one gruesome scene.

What do I have to do to get the eye? You tell Volo in your camp about the parasite that has infested your brain. He promises to look for solutions to help you. You lie down on your ear and after a long break you have a conversation with Volo again. He comes up with the glorious idea of operating on your eyes.
The moment has come. One of the strangest and most torturous scenes is about to happen. In his role as an amateur surgeon, Volo performs eye surgery on you that almost ends in a lobotomy. As he persistently stabs your eye, you must pass several trials to handle the situation.
During the horrific operation, you have the option to resign out of fear or let him continue the operation. Several times during the nerve-wracking scene you will be given the opportunity to opt out of the operation.
But hold on bravely. Volo feels guilty for not being able to remove the parasite and massacring your eye. He replaces it with a fake eyeball called “Volo’s Spare Eye”.
How to get Volo in your camp:
Without a doubt, for a small sacrifice you will gain an extraordinary ability that could make up for the loss of your natural eye. Therefore, allowing Volo to have eye surgery is definitely worth it.
The ability to spot invisible enemies provides an incredible benefit and adds depth to the game experience without any downside. In addition, the replacement eye looks damn cool.
